The Medicaid program in Wisconsin is run by the Department of Health and Family Services (DHFS).
Wisconsin provides Medicaid cochlear implant benefits for both adults and children that are roughly comparable to Medicare.
DHFS data shows that Medicaid paid for 29 cochlear implant procedures in 1999. This increased slightly to 33 in 2000. However, the average total charges jumped dramatically, a 68 percent increase from about $37,000 to over $62,000.
As of October 1, 2004, Wisconsin Medicaid reimburses $21,500 for the cochlear implant device (HCPCS L8614) and $6,0000 for replacement of the speech processor (L8619).
As of January 1, 2005, Wisconsin Medicaid reimburses replacement accessories as follows:
