Pacificare is one of the largest commerical health plans in the nation, serving over 3.4 million members. It also has among the most favorable coverage policies for cochlear implants. All Pacificare plans cover all costs associated with cochlear implantation (including aural rehabilitation) so long as the procedure is performed within the network. No copayment is required. Out of network cochlear implantation is generally non-covered. However, some individual HMO plans under Pacificare will provide benefits for the procedure out of network and require the patient to pay a copayment equal to 20-30 percent of the allowable charges. |
