The first two charts provide data from surveys sent by Hear US to all States in 2000. Please note that not all States responded, and the Medicaid programs for American Samoa, Guam, Puerto Rico, and the Virgin Islands were not surveyed.

You can also click here for a list posted recently by Self Help for Hard of Hearing that very briefly identifies cochlear implant Medicaid benefits by State. However, it is not clear what date this information was obtained.
