The Hawaii MedQUEST program provides cochlear implant benefits for both adults and children. Hawaii is the only state with an exception from Federal ERISA law, granting it the ability to mandate that all employers provide employee health care coverage. Through this exception and its Federal Medicaid waiver, Hawaii thus has nearly universal coverage for health care.

As of April 1, 2003, the physician professional fee for cochlear implant surgery under CPT 69930 is reimbursed at $972.60. Post-operative programming is reimbursed as follows:
