The Colorado Department of Health Care Policy and Financing provides Medicaid benefits for cochlear implant services to both children and adults.

Payment for the cochlear implant system (HCPCS L8614) under the 2003 CHCPF fee schedule is $13,188.74. Replacement of the speech processor (HCPCS L8619) is paid by invoice price, plus 20 percent for shipping and handling. Both codes require prior authorization.
